s/l attachment was a couple feet to his left, tensioning his left riser and resulting in a left 180. the toggle blowing may have had an imput on the off too, or possible a result from it.

Having the S/L attatchment way left would actually put more tension on the right riser, looks to me like the offheading was to the right. Either way a foolish thing to do, no sense packing symetrically if you yard the canopy out sideways.
The canopy looked like it had already rotated a lot before it even got tension on either side.
Back in the 1980s, jumpers used to have huge very heavy pilot chutes. Throwing the pilot chute to the side (since it would stay there and inflate there due to inertia) resulted in all sorts of weird openings.
